Civil Rights Self-Assessment Form
The Civil Rights Self-Assessment form provides an opportunity for the responses to the standards questions to be recorded in a simple, private manner. In addition, a portion of the Civil Rights Self-Assessment form allows for planning for modification of policies and procedures. The Civil Rights Self-Assessment form is available in a downloadable format to protect your school’s confidential information. The questions can be answered by editing the Word document saved on your computer or by printing the document then utilizing it hard copy format.
The form is accompanied by an ISBE exhibit list that provides examples of items that can be provided as evidence of compliance:
